Bereavement Leave - eligible for up to 32 hours of scheduled time missed for the loss of an immediate family member. (page 24)

Discounts - Hospital - 20% after 1 year of employment, Pharmacy - 10% off OTC, Purchasing - cost + 15% (page 19-20)

Employee Exercise Program - for employees only - %5 per month (page 25)

Extended Spending Plan - pre-tax dollars withheld from pay for medical and ay care expense through the calendar year. (page18-19)

Health Reimbursement Account - reimbursement of up to %1,000 of deductible expense met. (page 18)

Health, Dental, Vision, Prescription Drug Insurance - Paid for full-time employees and is available for family members.

Jury Duty Leave - employee will be paid at regular hourly rate for scheduled hours missed. (page 24)

Life Insurance - %10,000 provided to all employees working more than 35 hours per pay period. (page 32)

Voluntary Life Insurance - additional life insurance available for employee and familyi to purchase (page 23)

Long Term Disability - employees away from work more than 90 days qualify. (page 23)

Paid Time Off (PTO) - can accrue up to 160 hours per year, 80 hours vacation, 56 hours holidays, 24 hours sick. Used as approved by supervisor. (page 20-21)

Premium Holidays - Easter Sunday, July 4th, Thanksgiving, Christmas (starting with PM shift of Christmas Eve through the day shift of Christmas Day)(page 23)

Profit Sharing - must work 1,000 hours and be employed December 31st. Historically has been 2% (page 17)

401(k) - Auto enrollment of new employees at 3%. GMC will match up to 2% (page 17)

Unemployment Insurance - Required by state law (page 24)

Workers compensation Insurance - Required by state law. protects employee from paying medical bills due to work place injury/illness along with wages due to lost work time. (page 24)
