What is a Hospitalist and what do they do?A Hospitalist is a medical provider who specializes in the care of hospitalized patients. What are the advantages of a Hospitalist Program?
There are many advantages to a Hospitalist Program, including increased medical care efficiency and more immediate care. Many patients prefer Hospitalists because they are available 24/7 to ask questions regarding
medical care. Hospitalists are able to expedite the care process.
If I'm hospitalized, will I see my Primary Care Provider (PCP)?
If you are hospitalized, you will see a Hospitalist during your stay. The goal of our Hospitalists is to partner with your PCP to provide high quality and safe care.
Will my Hospitalist provide outpatient services after I'm discharged?
No, you will be referred back to your PCP for ongoing medical care.
How is continuity of care maintained between the inpatient and outpatient setting?
Communication is vital. Your PCP will be notified at the time of your admission and discharge. Your PCP will have access to your hospital records if affiliated with GMC, otherwise you may request that your records be forwarded to them after your discharge. If you do not have a PCP, our
team will assist you in arranging follow up care.
What will happen when I am discharged?
A Hospitalist will discuss your discharge medications and plan of care with you upon your discharge. They will also assist you in scheduling your first post-discharge office visit with your PCP.
